Nuclear war can also be started by unauthorised action, human error or sheer madness. There is moreover, a great danger of a person or a group of persons responsible for launching nuclear weapons going insane and deciding to launch a nuclear attack on the enemy, eliciting immediate retaliation and a real holocaust."
